Passo 2005 transmission problem O/D light blinking

I have Toyota Passo 2005 model. While driving my car normally I face a problem and suddenly my car’s O/D light start blinking and occurs a gear shift issue as well. I am facing the following issues in my car.
1. When car shifts from 1st gear to 3rd gear with a jump and accelerates then this problem comes in my car.
2. The car shifts to D o r R gear and makes a hard jolt when it is in idle state.
3. I think car moves to 3rd gear from 1st gear when a little acceleration is applied.
The issue resolved when if I turn the ignition off and restart the car. The issue comes again after 1 hour. And with the passage of time O/D light issue comes one day if happens this continuously. I have asked for suggestion and solution on this platform but unfortunately no one replied me to help to get rid of this situation. Kindly guide me to resolve the issue. Thanks.
