Passo Gearbox

I own a Toyota Passo 2004 model, the problem with my car is that, when I moves towards the R my car seems jerking and on the other side changing gear to D also feel jerk. What is the exact reason? Should I change the gearbox o r anything else. I have already changed the transmission oil recently.
Kindly tell me about a reliable mechanic somewhere in Rawalpindi.
