Transmission jerk passo 2007

I have transmission jerking issue in my Toyota Passo 2007 model. I started facing the problem from now, yesterday it was working properly. While shifting gear from P to R it gives a big jerk also at 40kmh speed when shifted to 2nd gear. One o r more time also jerked and felt a hammar striked at the speed of 40-50 kmh. I changed the rubber parts between engine and car chassis, the engine mounts to reduce the vibration of engine but not successful and engine transmission is still jerking. The oil of the transmission is also in the good condtion and I changed the oil 4-5 months ago. The mechanic asked me to change the transmission. So I want help and suggestion for this to fix this issue.
